Two Different Applications Depending on Your Firm Size

• Our EXPRESS APPLICATION: for qualifying firms with revenues of up to $1,000,000 over the last 3 year period. Select your state to review an application or purchase ONLINE and receive your policy within a couple of hours. If you can answer “true” to 9 eligibility questions you are eligible for the EXPRESS application. The rates are on the application and Prior Acts are included with a copy of your expiring Declarations page (assuming no gaps in policy terms). If you cannot answer “true” to the 9 eligibility questions on the Express Application, please complete our standard application and any applicable supplements.
